Transcript requests must be made in written form by using one of the methods listed below:

(All school locations and the Long Street office have copies of the request form if you wish to pick it up instead of downloading here.)
Please be sure to include all information requested as well as your signature and the processing fee (explained below).
- You may submit your prospective college’s transcript request form for processing, but it must include:
Full name (at the time you attended RSSS)
Date of birth
Name of last RSS school attended/graduated from
Year of last attendance/graduation
- You may also choose to submit a short written request to include:
Full name (at the time you attended RSSS)
Date of birth
Name of last RSS school attended/graduated from
Year of last attendance/graduation
All requests (whether mailed or delivered in person) must be accompanied by:
- Photo identification to protect against identity theft (i.e. driver’s license)
- Cash or money order for $5.00 (per official transcript) or $2.00 (per official immunizations only)
Please allow at least 3 business days for your request to be processed.
If a third party is to pick up your transcript, we must have prior notification, and the third party must provide us with photo ID before your transcript is released.
