School Health - Immunizations/NC Physical Assessment/Communicable Disease Info

Enrollment and Immunizations

  • North Carolina General Statutes require immunizations for every child enrolled in this state.
    Immunization Photo
  • No student may attend a school (pre K-12) without presenting a Certificate of Immunization (vaccine record).
  • The parent has 30 calendar days from the child’s first day of attendance to provide proof of the required immunizations.
  • Beginning with the 2016-17 school year, all students enrolling in a North Carolina public school for the first time, must have a physical assessment for enrollment. The form for the assessment: North Carolina Health Assessment Transmittal Form (pdf)
  • Students who enter the 7th grade after July 1 2015 must receive one dose of Tdap vaccine and one dose of Meningoccal vaccine for individuals who have not already received those vaccines.
